Loose lug nut mystery-HELP!

I have a 2002 Precedent 48 V. Last week I noticed all four lug nuts on my left rear wheel were so loose they could be removed with my fingers. I know I recently torqued them down correctly when changing that particular tire 3 weeks ago. I re-tightened them, and yesterday noticed one of the four nuts had loosened up---again! Could vibration from a bad rear wheel bearing, or even axle bearing, be causing this to happen and working the four lug nuts loose? I am not hearing any rear end howl or unusual noises.

Thanks for any help.

Vibration or even a bent wheel shouldn't come loose, it sounds like the wheel does not fit the application and won't seat properly.
What kind of wheels are you using and have you checked the fit?

The mag wheel is the same one that has been on the cart since new---4 years ago---never any problems. I tightened all four lug nuts down firmly, but I will go ahead and remove the wheel and check the seating/fit, etc. . Thank you.
